Your may lose your place at our practice if you
- Fail to make an appointment within the past 24 months.
- Fail to arrive, on 2 occasions within the past 24 months.
- Fail to provide 48 hours notice of a cancellation/ rebooking, on 2 occasions within the past 24 months.
- Are rude or aggressive towards any members of staff.
- Fail to settle any outstanding balances, in the agreed time period.
- Fail to settle any outstanding balances, in the agreed time period.
